No AV or Anti Spyware alone is completely effective. None. It takes a combination, secure browser, and prudent surfing to avoid infection.

Game cheat/crack sites, wares and keygen/serial sites, and of course many porn sites are all primary sources of infection. Outlook is another (via e-mail). At times I've seen ad servers pushing ads from questionable clients.

Houndog is right, anti-virus & anti-spyware are all reactive. They can't stop a future threat. These damn trojan horse pieces of malware, are a real PIA. We got a new one on a machine at work last week called "thinkpoint" that took me a half day to remove.

There are very few viruses written for mac & linux operating systems. They're not 100% virus-proof. We run all of those OS's at work though & I have yet to see a mac or linux virus.
